This notice describes information shared through JOVINA Skin Clinic and JOVINA Institute inquiries and appointment booking.

Information you choose to share

Clinic inquiries ask for your name, contact details, skin concerns, and optional appointment preferences and referral information. Institute inquiries ask for contact and professional details, your training or equipment interests, and any questions you choose to include.

Please share only what is needed for your inquiry. Do not include medical records, government identification, payment-card details, or other sensitive documents in the inquiry forms.

How the information is used

The JOVINA team uses these details to respond to you, discuss appropriate next steps, and coordinate appointments or professional training and equipment inquiries. Referral information helps us understand how people find JOVINA.

Submitting an inquiry does not book an appointment, reserve training, or complete a purchase.

Forms, booking, and outside services

Our inquiry forms are provided by Jotform. Online appointment booking is provided by Square. Information submitted through these services is processed within those services and made available to JOVINA to manage your inquiry or appointment. Each provider has its own privacy practices.

Links to Google Maps, Google reviews, Instagram, and WhatsApp take you to outside services. Their privacy practices apply when you use them. Opening your email or messaging app also uses the service you have selected.
